Allow users to upload a file when completing a survey
As a way to promote schools to submit their stories/events to the district website, I have created a survey asking for all the pertinent information. The only (and major) downside is that I am not able to ask them to attach/upload an image or file for our headline rotator. If we had this functionality, the process would be seamless and conducted in one area, as opposed to asking the user to take one extra step and email the file to me. Please include a file attachment question as one of the options in the Forms & Surveys tool.

File upload capabilities -- Add the ability for an end user to upload supporting documentation to a Form/Survey. Our old CMS (SchoolWorld) had this feature. Because this is not available in SchoolWires, we will have to find an alternative method for our campus admins to submit a Critical Incident Report to Central Office. We will continue to use our SchoolWorld form as long as possible, but this basic functionality should be added to SchoolWires system. Also need for students and parents to have the ability to submit documents such as homework and absence notes via the website.
I submitted a similar item but do not see it here - my idea: By using mailto coding the attachment would be e-mailed directly to the person configured and not utilize space on Schoolwires or Blackboard servers. The recipient's e-mail address would be configured in the survey and reference the survey number or title in the subject of the e-mail. Attachments could be limited (but not required) to graphics files (jpg or png) and pdfs. This would be huge for us in our translation department for submitting items to be translated and transportation department for submitting bus pass applications where a photo of the student is required as well as other use cases.
This would be very helpful.

I'm still searching the web for how to do this, but if this feature isn't available yet, I third the request.

Our district would like to do the same thing:
We want to create an extensive survey for students interested in joining our Yearbook Committee and ask that they upload some examples of their work.
